Appointment Checklist

If you are interested in becoming a patient:

• Call (313) 494-6700 to schedule a screening appointment.
• Bring all x-rays from the last five years.
• Complete the enclosed health questionnaire and patient information sheet.
• Bring ALL current medications with you.
• Bring all insurance information, your insurance card, and photo identification.
• You will need to bring $40.00 for necessary x-rays.
• If you are under the care of a physician, please bring the name and address of your physician.

Things to Remember

If you are interested, we hope to see you soon. However, not all patients are accepted to the student clinical program. Patients may also request a staff or faculty dentist for their care.

Because our students provide you care, your care will take longer in the student clinical program than in a private dental office.
