Admission requirements

• Dental Hygiene Graduate from accredited DH Program
• Overall GPA of 2.5 or above in academic coursework
• Successful completion of National Board Exam
• Current DH license
• Current CPR certification

Application Process

1. Complete the online application

• Notify Judi Luxmore once the application has been submitted.

2. Send official copies of all transcripts from all colleges and universities attended to:

3. Send copies of DH license, National Board scores, and current CPR certification
