Registration Information

Tuition-paid registrations are processed in the order in which they are received and will be acknowledged in writing (if time permits) or electronically through e-mail.

Online registration powered by ABC Signup and Authorize.net is now available using the buttons on the course description pages. This is the preferred method of registration and payment can be made by credit card or check.

Registration Form
A registration form is available online for print. Please make as many copies as needed. However, use one form per person, per course.

Refund and Cancellation Policy
Unless otherwise posted, a full refund less $25 will be granted if notification of cancellation is received by this office at least two weeks prior to the start of the course. Refunds cannot be given for cancellations received once the course has started.

*There will be a $100 administration fee charged for cancellation or rescheduling of the Local Anesthesia Administration for the Dental Hygienist course, including the clinical session, once the participant is registered in a lecture and laboratory session.
